Heritage, Heritage Prime, and Heritage Signature are Premera provider networks. They help determine which doctors, hospitals, and medical groups employees can use at in-network rates.

Heritage offers the broadest access of the three. One major difference is access to Providence and Swedish: Heritage and Heritage Signature include these providers, while Heritage Prime generally excludes them. Both Prime and Signature limit CHI Franciscan participation to Kitsap County.

Heritage vs. Heritage Prime: the Providence and Swedish difference

For employees receiving ongoing care through Providence or Swedish, a move from Heritage to Prime can affect both where they receive care and what they pay. Keeping Heritage may help preserve access to their existing care team at in-network rates.

Prime’s hospital options include Harborview, University of Washington Medical Center, Seattle Children’s, Virginia Mason Medical Center, Overlake, EvergreenHealth, and several MultiCare hospitals. For employees who already use these providers, Prime may offer a familiar set of care options.

Heritage Signature includes Providence and Swedish

Employees using Heritage Signature have access to Swedish First Hill, Cherry Hill, Ballard, and Issaquah, along with Providence Regional Medical Center Everett and Providence St. Peter Hospital. The network also includes Kadlec Regional Medical Center and Pacific Medical Centers.

Heritage has a larger hospital network overall: 119 hospitals compared with Signature’s 112, based on the May 2026 figures in Premera’s 2027 small-group guide.

CHI Franciscan in Kitsap County and Virginia Mason access

For both Prime and Signature, CHI Franciscan participation is limited to Kitsap County. Options include St. Michael Medical Center and Franciscan Medical Group’s Kitsap County clinics.

Virginia Mason Medical Center accepts both Heritage Prime and Heritage Signature. Within Virginia Mason Franciscan Health, network participation varies by hospital or medical group and location.

How the network fits into the plan name

In Premera’s direct small-group offering, Choice plans use Heritage and Balance plans use Heritage Signature. BHT has its own benefit plan names. When reviewing a BHT proposal, ask your broker to identify the network alongside each plan’s premium and benefits.

The PPO designation describes a plan that generally covers both in-network and out-of-network care, with higher costs outside the network. Compare each plan’s deductible, copays, coinsurance, and out-of-network benefits alongside its provider access.

Why would an employer choose a smaller network?

A more selective network can help lower premiums through negotiated provider rates. Providers participating in Heritage Prime accepted lower rates to join that network. Heritage Signature is also designed to offer a lower-cost alternative to Heritage.

Start with the savings shown in your actual quotes. If employees’ doctors and hospitals participate in both options, the lower-priced plan may be a good fit. If switching would put a regular specialist or preferred hospital outside the network, weigh the savings against employees’ added costs and the disruption of changing providers.

A little history behind the names

Prime and Signature have been around for more than a decade. Premera was using both names in 2014, when it announced that additional Swedish facilities would join those networks for 2015.

For 2020 individual plans, Premera changed the network’s name from Heritage Signature to Individual Signature, keeping the same network at the time. Heritage Signature continues as the name used with its direct small-group Balance plans.

Before choosing a plan

Give employees the exact network name and time to check their doctors, specialists, hospitals, and regular care locations in Premera’s provider directory. Confirm participation for the plan’s effective date and the location where care will be provided.

BHT helps Washington employers compare coverage with their budget and employees’ needs in mind. Work with your broker and BHT to review the premium, benefits, and provider network together, so the choice reflects both what your business pays and where employees can get care.
