A clinician who gets to know you

You’ll work one-on-one with a Nice clinician to develop a personalized care plan. According to Nice, patients see the same specially trained provider at each visit throughout their program. That continuity gives you someone who knows your goals and can follow your progress over time.

Depending on your needs, support may include education, follow-up visits, lifestyle guidance, medication management, and coordination with other Nice services. Nice also offers care navigation to help with scheduling appointments and working toward individual goals.

Four programs built around different health needs

Nice’s Specialized Programs currently cover four areas:

  • Type 2 diabetes: Personalized support to help you understand and manage diabetes, work toward healthier A1c levels, monitor your condition, and build sustainable nutrition and movement habits.
  • Hypertension: Guidance to help you understand and monitor your blood pressure, take medications consistently and safely, and develop habits that support long-term heart health.
  • Weight management: Judgment-free support centered on your health, lifestyle, and personal goals, including a realistic nutrition and movement plan.
  • Perimenopause and menopause: Support for physical and emotional changes, symptom management, changing cardiovascular health risks, and quality of life.

Your clinician can help you decide whether a program is a good fit and what your next steps should look like. You don’t need to arrive with every question figured out.

What Nice’s early results show

In a recent newsletter, Nice shared preliminary clinical outcomes for participants in three of its programs:

  • Hypertension, at six months: An average blood pressure reduction of approximately 5 mmHg.
  • Diabetes, at four months: An average A1c decrease of 0.98 percentage points.
  • Weight management, at four months: An average weight loss of 29.2 pounds, or roughly 12% of body weight.

These are encouraging early averages reported by Nice, and individual results will vary. They don’t yet establish long-term outcomes or cost savings. Nice says sustaining progress over time is its priority.
